southwest corner of Alabama. Efforts will also include in-person
enrollment support and outreach to diverse populations.

AgeOptions
will focus on increasing benefit enrollment in the suburban Chicago
area with the support of current program initiatives that include
strong networks: The Red Tape Cutter Program, Make Medicare Work
Coalition and the ADRC. Twelve local benefit enrollment centers will be
established throughout suburban Cook County that will target
underserved groups, people with disabilities and ethnic populations
through community outreach efforts.

Green River Area Development District (GRADD)
Owensboro, Kentucky
GRADD
will work with faith-based partners, local Area Agencies on Aging
(AAAs) and other community-based partners to provide outreach and
application support at Benefit Enrollment Centers throughout the
largely rural, approximately fifty county region that comprises the
western half of Kentucky. Through the use of faith-based volunteers,
events with on-site enrollment assistance, and community-based
referrals, GRADD plans to take comprehensive application assistance to
locations where those likely eligible for benefits congregate.

Maine SHIP, Office of Elder Services

The
Maine State Health Insurance Assistance Program (SHIP) will reach out
to isolated populations in 13 very rural counties by providing benefits
and enrollment assistance via three strategies: conveniently located
information kiosks equipped with touch screen technology, 2 mobile
offices and a variety of community outreach events. A peer-to-peer
model will be employed through the use of Senior Community Service
Employment Program and Volunteers of America Northern New England
members as well as other trained community volunteers.

strengthen interagency coordination and maximize technology resources
to create an improved benefits enrollment system.

Elder
Law of Michigan's Legal Hotline for Michigan Seniors state-wide senior
legal hotline will provide telephone-based benefit enrollment
assistance. Elder Law will collaborate with community partners in
Detroit and Pontiac that will be equipped to provide person-centered,
community-based application assistance, well-qualified client leads and
access to vulnerable populations in urban locations.

Helena, Montana
The
Montana Department of Public Health & Human Services project will
cover a seventeen county region and will focus on outreach to rural
frontier counties and Reservations. Benefits outreach and enrollment
efforts will be accomplished in conjunction with ADRC sites in four
AAAs and one Council on Aging. The Department of Public Health &
Human Services will also coordinate with state agencies to identify and
screen beneficiaries enrolled in other assistance programs.

a grassroots and community-based organization that enhances
opportunities for people with disabilities to direct their own lives,
CIDNY will work to streamline benefits enrollment with a comprehensive
partner network serving the five borough region, including three
agencies that operate New York City's funded senior centers. Benefits
education and enrollment will be conducted on-site to diverse
populations in familiar settings throughout the metropolitan region.

Western Reserve Area Agency on Aging
Cleveland, Ohio
Through
a comprehensive array of core partners including the Benjamin Rose
Institute, the Cleveland Department of Aging, Fairhill Center,
211/First Call for Help and the local ADRC, Western Reserve Area Agency
on Aging's efforts will serve to strengthen existing benefits-focused
collaborations to ensure a coordinated system for providing benefits
assistance throughout the greater Cleveland area. Four enrollment
centers (including one at the public hospital system and one at the
local Center for Independent Living), a call center and transportation
services will provide comprehensive, community-wide benefits enrollment
support.

Benefits
Data Trust's BenePhilly Initiative project, a collaboration between the
Philadelphia Mayor's Office, the Pennsylvania Department of Public
Welfare, the Pennsylvania Department of Aging and other community-based
partners, will provide centralized application support utilizing an
integrated city-wide referral system. Through the use of lists gathered
from partners, direct mail solicitation, referrals and outbound
calling, BenePhilly will provide targeted enrollment support to those
likely eligible for the State Prescription Assistance Program, the
Medicare Part D Low-Income Subsidy, the Supplemental Nutrition
Assistance Program (or SNAP -- formerly known as Food Stamps), the
Low-Income Home Energy Assistance Program (LIHEAP) and other state
benefit programs.
